For new patients if you are diagnosed with periodontal disease this fee will be increased to $494.25. A separate contract form will need to be signed. We will assess your oral health and determine diagnosis at your first visit.
Payments for services are due at time of service. It is a dental membership plan and not dental insurance. It is an annual, one year contract between the patient and South Ridge Dental.
This membership plan is not good at any other location and can not be supplemented with an insurance plan. If our dental office refers you out to another dental office for treatment, this plan will not be applicable.
If dental treatment is due to an accident where worker's compensation, disability compensation, or a lawsuit will be involved, this plan will not be applicable to those services.
Limitations and exclusions may apply. The discount does not apply to products sold in the dental office. Rates are subject to change annually.
This is a non-refundable fee, family members can not be substituted in place of the member. No refunds will be given if the patient chooses not to use their membership benefits.
